Culturally Competent Community Health Workers Improve Outcomes and Reduce Inpatient Utilization Among Inner-City HIV/AIDS Patients
The Prevention and Access to Care and Treatment project uses specially trained community health workers to deliver culturally competent, home-based support services to help HIV-positive patients in the inner city.

Medical Outreach to HIV Patients in Unstable Housing Increases Use of Care and HIV-Related Medications and Improves Patient Perceptions of Quality
The Medical Outreach Program enhances services provided to unstably housed, minority individuals with HIV by including physicians on the community outreach team who provide general medical consultation, triage, acute care, evaluation, and prescriptions for needed medications.

Rapid HIV Testing Program Enhances Access to HIV Education and Screening for At-Risk African-American and Hispanic Youth
The Rapid HIV Testing Program provides sexual health education and counseling to at-risk youth, rapid HIV testing, and individual counseling to those who test positive; the program is conducted in community-based settings that are easily accessible to the target population.
